We’re thrilled to support you in planning an unforgettable Outdoor Education Leadership Trip to Camp Muskoka. To ensure your experience is smooth and successful, we’ve organized this page into easy-to-navigate sections. Explore the resources below to find everything you need to make your trip a success.
All forms are available as fillable PDF’s. If your school chooses to use only digital forms, please be sure to advise your parent community to “Save As” the student’s first and last name PLUS the name of the form so they may be easily managed.
Step 1: 6 Weeks Before Your Visit to Camp Muskoka
1.) All of the resources, forms, planning and program information for teachers can be found below.
2.) Plan an information session for parents at your school using the provided Camp Muskoka informational video.
3.) Send the information package home with students with the following forms and a return date of 4 weeks before the visit date:
- Parent/Guaridan Letter
- Informed Consent/Permission Form
- Student Registration Form
- Packing List
4.) Confirm who the Teacher Supervisor will be, as well as the Parent/Guardian Supervisor(s).
Step 2: 4 Weeks Before Your Visit to Camp Muskoka
1.) Verify all student forms have been completed and returned from parents/guardians:
- Parent/Guardian Letter
- Informed Consent/Permission Form
- Student Registration Form
2.) Verify all money has been collected from students. Staff may request to access the Feed All Four Fund in the event that a family is unable to cover the cost of the trip. Please deposit the collected money to GL 0-07-026-1-060-000-000.
3.) Verify the names and numbers of Supervisors attending (one male, one female):
- Police Record Checks are required for Parent/Guardian volunteer(s)
- Complete and collect the Teacher/Supervisor Registration Form from the voluneteers
4.) Complete the following:
- Emergency Dietary & Medical Summary Form
- Final confirmation of the number of students and names of Supervising staff
- Scan copies of forms to prepare for sending to Camp Muskoka
Step 3: 3 Weeks Before Your Visit to Camp Muskoka
1.) Camp Muskoka staff will call the teacher superviser to connect and complete the Pre-Plan, the following items are to be discussed:
- Select Program Activities for your trip using the blank itinerary as a reference.
- Submit scanned student forms and staff/supervisor forms, Emergency Dietary & Medical Summary form to Camp Muskoka staff, oe@campmuskoka.com
- Discuss any required accommodations (health, dietary, etc.) as well as any emergency medical conditions noted on the Emergency Dietary and Medical Summary Form.
- Disclose any student's who may not be filmed or photographed.
